Building Use Application To request use of the building, please fill out and submit this form at least 1 MONTH prior to the event.
(If you are approved, it will be necessary for your insurance provider to give a valid Certificate of Insurance listing Covenant Presbyterian Church of Chicago as additionally insured with respect to your event. You will also be required to submit a refundable deposit and you will be charged a fee for your use of the property. Please contact the property manager if you have any questions.)
